Try the latest version and check if the problem persists.
These games always say Landlady, Tenant, etc. The “step/not quite actually family” bit is in place within the writing to get around paetron and other hosting site rules about taboo content like Incest. There is a patch in the links labeled “incest,” so that it will re-write the parameters to be “blood related.” It’s a bit of a PITA but that’s always been the way it’s worked since I started playing these games. (Originally I did find it odd as well, but you get used to it)

It is script .rpy file. How is it mac compressed version?
We get TheIsland-0.9-mac-Compressed .zip from MEGA with that patch. Something is wrong on your end if your getting something we are not mate.
